Best Comms Setup Empty Best Comms Setup

It seems in the world of field communications there are many different styles of comms setups. Some people have just the straight radio and use the integrated mic and speaker. Other have the police or fire style hand mics that plug into the radio. While still others take the military approach with the headsets and boom-mics. Which approach do you prefer and why?
